Vitality Unlimited manages one transitional group home, the Sage House in Elko, Nevada, to help individuals recovering from drug and alcohol abuse become self-sufficient. Counseling and continued treatment are a condition of residency.

The safe and supportive environment is designed to help up to eight recovering patients practice healthy living skills, become more adept and comfortable living independently, find and maintain employment, and receive additional treatment as needed.

To qualify for admission, applicants must be 18 or older; have completed intensive residential treatment; demonstrated the need for transitional low-cost housing as a step toward self-supported independent living; meet criteria for co-enrollment in Level I treatment services.

The resident and his or her counselor create a personalized treatment plan that includes this transitional living phase. Residents must also attend regular AA or NA meetings and individual/group counseling sessions, as advised by their counselor to assist them in maintaining sobriety and experiencing a successful recovery.

To be discharged from transitional housing, the individual must meet at least one of the following criteria: Client meets discharge criteria for ASAM PPC-2 Level I or II service for adults; client does not continue to be a patient in a state-approved Level I or II service for adults conducted by the operator of the transitional housing service; client requires more intensive supervision than that provided by the service; or clinically appropriate alternative arrangements for housing of the client are available to the client.
